Under general direction and in collaboration with the multidisciplinary Radiation Oncology team, the Nurse Coordinator provides expert nursing care for Radiation/Oncology patients with a variety of complex treatment options. The nurse coordinator serves as a contact point for new patients, family members, referring physicians, department physicians, RTT's and other Radiation Oncology team members as well as a contact point for the coordination and facilitation of the care of the patient in specialty and procedural areas outside of Oncology. The Nurse Coordinator provides leadership for collaborative and multidisciplinary patient care with emphasis on education and continuity of patient care for the entire period of treatment and survivorship. The Nurse Coordinator is responsible for monitoring and coordinating the care of Radiation Oncology patients to ensure they reach their maximum health potential before, during and after treatment.
Specific daily responsibilities include triaging patient calls and lab results, coordinating studies and specialty referrals, notifying Providers of any upcoming orders needed, providing initial and ongoing patient education. Directing the coordination of care for the patient across disciplines, and care continuum as needed. Nurse coordinators will focus on new patient referrals, coordinating their care through multimodality treatments and ensuring they move efficiently and safely through their care journey. This position will support the Radiation Oncology team by communicating with patients on behalf of providers. The nurse will coordinate changes to treatment plans as delegated by the provider. The nurse coordinator will educate patient on any changes and document in the EHR. The final salary and offer components are subject to additional approvals based on UC policy.Your placement within the salary range is dependent on a number of factors including your work experience and internal equity within this position classification at UCSF. For positions that are represented by a labor union, placement within the salary range will be guided by the rules in the collective bargaining agreement.
The salary range for this position is $91.20 - $118.34 (Hourly Rate).
